This plan offers a $50 annual deductible waived for preventive treatment. In-network coverage 100% for preventive care, 100% for basic care; 60% for major services and 50% for ortho. Out-of-network coverage is 100% for preventive care, 80% for basic care, 50% for major services and 50% for ortho, A breakdown of which services fall in which categories is available. The plan is subject to a calendar year maximum of $1000 per person. Dependants are covered to age 20 with full time students covered to age 26. New enrollees are subject to a 12 month waiting period for major services.

Members with less than 5 active, full time Employees must have 100% participation. Members with more than 5 active, full time employees must have 75% participation.

Individual: $67/month
Family: $130/month
